During the summer, working papers are available in the main office.  During the school year, working papers can be obtain from Mrs. Rambo at the front desk.

Listed below are the sections that need to be completed before the form can be certified by DHS:

Section A - Personal Information must be completed and signed by a parent/guardian.

Section B - Employment Information must be completed and signed by the employer after employment has been secured.  Signature of a parent/guardian is also required in this section.

Section C - Physician's Certification must be completed and signed by a physician after a physical exam.

Sections D, E, F and G will be completed by Mrs. Rambo, Mrs. Newman, or Mrs. Schenski.  After the entire form is finished, the student will receive the pink copy of the form to return to the employer.
